Leaving Hospital

Discharge Liaison Service

We have a Discharge Liaison Service who will facilitate your discharge from hospital if needed.  Alongside the ward staff, they will liaise with any community based teams that may have input into your on-going care when you leave hospital.

Discharge liaisonOnce a decision has been made about your discharge this will be discussed with you. Please be aware it may take several hours to sort out your discharge medicines and all the relevant paperwork before you can actually leave the hospital.

When you are ready to be discharged, we will arrange for you to be transferred to the discharge lounge - this is a comfortable place for you to wait until your relatives or friends can pick you up.

If you are not being discharged to your home you and your family will be consulted to find the most appropriate accommodation.

You will be given a copy of the discharge summary to take home along with any medicines you require. In addition you can request a copy of the letter sent to your GP.

If you are able to make your own travel arrangements please inform staff.

All patients who meet eligibility criteria for hospital transport will have their transport arranged by the hospital.

Medications

Leaving hospitalThe nurse, pharmacist or pharmacy technician will discuss any changes to your medications before leaving hospital. Any newly prescribed medicines will be supplied to you before you leave and we will write to your GP to let them know of the changes. If you require further supplies after leaving the hospital you will need to book in to see your GP.
